Medical
errors are the fifth leading cause of death in the United States,
according to the December 1999 Institute of Medicine (IOM) study.
The study asserts, “Designing a ‘culture of safety’ within
healthcare organizations demands the creation of systems that respect
human limits in process design, includes use of constraints and forcing
functions, standardizes patient records, and promotes uniform standards
for connectivity, terminology, and data sharing.” The
